You may encounter various optional bosses while exploring various dungeons or locations in Realm of Shadows in the Elden Ring: Shadow of the Erdtree DLC. Defeating them will reward you with a certain amount of runes and some weapons, recipes, or other interesting items that might prove useful.

Chief Bloodfiend Location
Chief Bloodfiend is located at the end of the Rivermouth Cave dungeon. Head to the Temple Town Ruins Site of Grace to reach this location and go south. Climb along the edge of the cliff to the tombstones and descend. Continue west along the wall until you reach the entrance to Rivermouth Cave.
You can take an alternative route if you don't have the Temple Town Ruins Site of Grace. Teleport to Castle Front. Summon Torrent, pass between the tents, and follow the path between the barricades. Continue along the road until you see a large rock on your left, stay close to it, and descend the cliff using the ledges.
Ahead, you'll see a flower near the cave entrance. Staying on the left side, you’ll reach a Site of Grace; activate it and continue on, turning right as you exit the cave. Run straight along the river until you reach its far bank, where you'll see a cave in the rocks—this is the entrance to the Rivermouth Cave dungeon. After passing through it, you’ll find Chief Bloodfiend.

How to Prepare for Chief Bloodfiend
Based on the boss's stats and abilities, you can prepare accordingly to face and counter Chief Bloodfiend. First and foremost, equip talismans that can protect you from various types of enemy attacks.
For physical damage, you can use the Dragoncrest Greatshield Talisman, which reduces it by 20%, and for bleed damage, the Stalwart Horn Charm, Tottled Necklace, or the Stanching Boluses consumable. For fire damage, use the Flamedrake Talisman.
Depending on your build and preferred effects, bring additional talismans that enhance your abilities, whether it's strength from certain types of magic or incantations, dexterity, increased damage when health is low, etc. Adapting your build for the boss isn’t mandatory, as the boss is fairly simple, but these tips will help you better prepare.
Additionally, during the fight with Chief Bloodfiend, you can summon an Ash Spirit to assist you. The Mimic Tear Ashes and Black Knife Tiche Ashes are particularly effective. If you’re playing in multiplayer, your partner can also join the battle.
Battle with Chief Bloodfiend
Upon entering the arena with Chief Bloodfiend, the boss will turn and perform a jumping attack toward you, swinging a club. After that, he’ll follow up with several consecutive strikes at the landing point. To avoid this attack, roll forward under the boss, positioning yourself behind him, but don’t rush to attack unless you’ve secured a good position to avoid getting hit by his weapon.
Next, Chief Bloodfiend will stomp forward toward you, so try to keep your distance to avoid taking more consecutive hits and losing balance. When the boss stops, there’s a window to attack him.
f Chief Bloodfiend does a single downward club strike or a swing, you can successfully dodge it by rolling. Similarly, you can dodge his hand swing, which is also easy to evade.
you can use anti-bleed pills or a talisman to counter this, it’s better to roll to the side to dodge the spits. Another troublesome attack is his claw swipe, which causes a series of fiery explosions in a nearby radius.
Most of the boss's attacks are close to mid-range, involving his club, fists, feet, or spits. By keeping a long distance, you can use spells or ranged weapons to deal with the boss, which can simplify the fight and allow you to avoid the majority of his attacks.
That said, remember that some moves, like his jump attack or giant charge, can close the distance, so be mindful to avoid getting caught off guard. In close combat, roll at the moment you see Chief Bloodfiend preparing to attack. Usually, rolling toward the boss or behind him will suffice.
Rewards for Defeating Chief Bloodfiend
After defeating Chief Bloodfiend, you will receive the following rewards:

Once you’ve collected these rewards, you can safely exit the dungeon using the nearby teleport, which will take you back to the beginning of the location.
