2. Northeast of Scorched Ruins: This fragment can be obtained by destroying the Pot Shadow in the cemetery northeast of Scorched Ruins. Since these enemies disappear quickly, you need to act even faster. If you fail, reload the area.

3-4. Church Of Consolation: Head south along the main road through Gravesite Plain. At the end of the road, you'll find the Church Of Consolation. Two fragments are located at the foot of the statue inside.
